In the deleted "third timeline" (the one from the 2016 Special Rehearsal Edition script, where Albus and Scorpius block Cedric in the maze, Harry dies in the graveyard, and Voldemort wins quickly and decisively), Bellatrix Lestrange is alive, triumphant, and very visibly pregnant with Voldemort's child — Delphini.

When Scorpius returns alone to the alternate 2020 Hogwarts, the school is under full Death Eater control. Voldemort himself enters the great hall for a public assembly on Voldemort Day. Bellatrix walks in proudly at his side, heavily pregnant (clearly in the late stages). She is wearing Death Eater robes modified to accommodate her pregnancy, and she is described as glowing with pride and power. Voldemort publicly acknowledges her and the coming child (Delphini), presenting it as the future heir of Salazar Slytherin. The students and staff are forced to cheer; Scorpius is horrified.
